Frequently Asked Questions

  • Applicants for licensure as a music therapist must complete and submit a paper application 
  • Applicants must submit the following information:
    • Completed application
    • Non-refundable application fee of $100.00
    • Completed criminal background check
    • Verification of licensure from any/all states where licensed as a music therapist
    • Verification of certification from the Certification Board for Music Therapists or the National Music Therapy Registry
    • Any other additional information requested by the Board
  • Applicants should register with COGENT Systems prior to submitting their application for licensure (

Applications are typically reviewed in less than fifteen business days.  Applications are reviewed on a first in/first out basis.  To ensure the most efficient processing time, please submit a complete application using the instructions contained in the application packet. 

All address changes must be reported within 30 days after the change.  When making address changes please remember to update your email address.  Changes may be submitted by clicking here.

  • Lost or stolen pocket cards should be reported to the Board immediately. 
  • You may print a copy of your pocket card by clicking here.
  • If you would like to order a pocket card you may submit your written request to 237 Coliseum Drive, Macon, Georgia 31217.  Please include a check or money order payable to the Professional Licensing Boards in the amount of $25.00.  Be sure to include your license number with your request.
  • Licenses expire March 31 of even numbered years. 
    • Licenses may be renewed online by visiting the website and completing the following steps:
      • Click on "License Renewal"
      • Follow the onscreen instructions to login and submit your renewal information
      • Submit secure and verifiable documentation of US citizenship as instructed on the online renewal site
      • Submit the non-refundable renewal fee of $50.00
    • You may request a paper renewal form by contacting our office at 478-207-2440.  Applicants who wish to complete a paper renewal form must submit the following information with their paper renewal form:
      • Complete all demographic information
      • Answer required renewal questions
      • Submit secure and verifiable documentation of US citizenship as instructed on the online renewal site
      • Submit the non-refundable renewal fee of $50.00
  • Applicants must register with COGENT prior to visiting a fingerprint site.  No data collection or registration will be permitted at the fingerprint collection sites and fingerprint services will not be provided to applicants who have not registered.
  • Applicants may register with COGENT systems online or by telephone.
    • Online registration is available twenty-four hours a day, seven days a week by visiting
    • Registration by phone is available Monday through Friday, 8:00am to 6:00pm EST by calling 1-888-439-2512.
    • When registering, please use the following information:
      • ORI Number – GA923001Z
      • Verification Number - music
