Official Certificate Request Form
Guidelines For Certificates
The Secretary of State’s Office provides Outstanding Georgia Citizen, Honorary Georgia Citizen, and Outstanding Business certificates at the request of members of the Georgia General Assembly. These certificates are designed to be given to the most distinguished Georgia citizens and visitors to our state.
- The recipient must be a Georgia citizen to receive an Outstanding Georgia Citizen certificate and a non-resident to receive an Honorary Georgia Citizen certificate.
- In addition to the Certificate request form, please include an explanation of why the intended recipient is worthy of this honor.
- The Secretary of State’s Office provides Outstanding Georgia Citizen certificates, Honorary Georgia Citizen certificates, and Outstanding Business certificates, and the template should not be modified, duplicated, or distributed.
- An individual/business cannot be awarded multiple certificates. In addition, the Secretary of State’s Office will not replace lost, stolen, or damaged certificates.
- Businesses that are awarded the Outstanding Business certificate generally have recently achieved specific accomplishments that the Secretary of State’s Office may honor with the presentation of the certificate, and documentation of such may be required. If they are registered with the Secretary of State’s Office, they must be active and compliant.
- Certificates should not take sides in matters of political or social controversy.
- All requested recipients are subject to review by the Secretary of State’s Office and the office reserves the right to deny or modify requests that fail to meet guidelines.
- All certificates are hand signed by the Secretary of State. For this reason, please allow 20 business days for certificate production.
- The Secretary of State’s Office can provide each member of the General Assembly with up to 5 certificates per month.
- Certificates must be requested by members of the Georgia General Assembly or other elected officials. The Secretary of State’s Office will not honor certificate requests that are not requested or sponsored by an elected official.
