Secure and Verifiable Documents

Important Information Regarding Secure and Verifiable Documents, including Affidavit of Citizenship and Qualified Alien Documents.

Secure and Verifiable Identity Document Act

The "Secure and Verifiable Identity Document Act" requires applicants for licensure to submit an Affidavit of Citizenship and a secure and verifiable document regarding their United States' citizenship status.

Many licensees have questioned whether or not they would be required to submit the secure and verifiable documentation. The answer is "Yes."

To meet this requirement, be prepared to submit the following documents with your application:

Secure and Verifiable Documents

Submit a completed, signed and notarized Affidavit of Citizenship. You may click on the following link to use this standard version: Affidavit of Citizenship Form . If you elect to use this form, please be sure to CLEARLY PRINT your name and the license type that you are trying to obtain so that our staff can properly process your information.

If you are a United States citizen, submit a copy of one of the acceptable secure and verifiable documents (a complete list may be found by clicking here). The easiest option is to simply submit a copy of your driver's license or United States passport.

When submitting your documentation please make sure that the image is clear and readable.

If you are a qualified alien, submit a copy of your qualified alien documents which grant you the legal right to work in the United States. This document should include either your Alien number or your I-94 number and, if needed, SEVIS number. If submitting a copy of a card, please submit copies of the front and back of the card.

When submitting your documentation please make sure that the image is clear and readable.

Be sure to indicate which type of license you are applying for. If you have already received your license, include your application number.

Court Documents

Applicants who answer YES to application questions concerning criminal history or disciplinary actions taken against them must submit certified documentation of court dispositions, agency orders, or any other documentation to provide a complete answer to such questions.  Failure to provide this information will result in processing delays and may be grounds for disapproval of the application by the Board. In the event your court case file no longer exists, you must submit documentation from the court stating that fact.

Where To Send Your Documents
