Secure and Verifiable Identity Document Act
The "Secure and Verifiable Identity Document Act" requires applicants for licensure to submit an Affidavit of Citizenship and a secure and verifiable document regarding their United States' citizenship status.
Many licensees have questioned whether or not they would be required to submit the secure and verifiable documentation. The answer is "Yes."
To meet this requirement, be prepared to submit the following documents with your application:

Court Documents
Applicants who answer YES to application questions concerning criminal history or disciplinary actions taken against them must submit certified documentation of court dispositions, agency orders, or any other documentation to provide a complete answer to such questions. Failure to provide this information will result in processing delays and may be grounds for disapproval of the application by the Board. In the event your court case file no longer exists, you must submit documentation from the court stating that fact.
