All address changes must be reported within 30 days after the change. When making address changes, please remember to update your email address. Changes may be submitted by clicking here.
Lost or stolen pocket cards should be reported to the Board immediately.
If you would like to order a pocket card you may submit your written request to 237 Coliseum Drive, Macon, Georgia 31217. Please include a check or money order payable to the Professional Licensing Boards in the amount of $25.00. Be sure to include your license number with your request.
You may request a paper renewal form by contacting our office at 478-207-2440. Applicants who wish to complete a paper renewal form must submit the following information with their paper renewal form:

Answer required renewal questions
Submit secure and verifiable documentation of US citizenship as instructed on the online renewal site

All licensees must complete a minimum of for 40 hours of continuing education in a program approved by the Certification Board of Music Therapists. Continuing education credits for the 2022 renewal period must be completed between April 1, 2020 and March 31, 2022.
