Identification Card for the Georgia Board of Massage Therapy

The Georgia Board of Massage Therapy offers licensed Massage Therapists the opportunity to purchase a durable plastic Identification Card.

Licensees must complete an Identification Card Request Form in person from one of these Secretary of State locations from 8:30 am to 4:30 pm:

• Atlanta at 2 MLK Junior Drive, Suite 802, West Tower of the Sloppy Floyd Building
• Macon at 237 Coliseum Drive
• Tifton at 238 East Second Street

• A valid picture ID must accompany the Identification Card Request Form.
• Staff will confirm information based on the licensee’s record before issuing an Identification Card.
• The licensee is responsible for verifying the accuracy of their information on the Secretary of State’s website prior to submitting an Identification Card Request Form.
• Identification Cards have an expiration date which mirrors the license expiration date.
• Expired Identification Cards are not valid as proof of licensure.
• The purchase of an Identification Card is not a renewal of a license. Licenses must be renewed in accordance with State Law and Board Rules.
• A new Identification Card may be purchased under the following circumstances:
  • Expiration of the Identification Card;
  • Change in license status; or
  • As a replacement for a lost or damaged previously issued Identification Card.
• The Identification Card should not be altered or modified by the licensee. Doing so will invalidate the Identification Card.

The Identification Card fee is $50.00.

Additional cards or replacement cards are subject to the same fee.

Payment for Identification Cards must be by credit/debit card at the time the Identification Card is issued.
